WHO?  PARENTS OF KINDERGARTEN AGE CHILDREN AT LEAST 5-YEARS

         OLD BY AUGUST 15, 2018:  


WHERE?  LYNCHBURG ELEMENTARY SCHOOL (SCHOOL NURSE’S OFFICE)

WHEN?  APRIL 10th – 12th    (DURING THE HOURS OF 8:30 UNTIL 11:30)

UPON REGISTERING, YOU MUST PROVIDE DRIVER’S LICENSE WITH CURRENT MOORE COUNTY ADDRESS FOR PROOF OF RESIDENCY.

YOU MUST BRING THE FOLLOWING DOCUMENTS

                                IN ORDER TO REGISTER YOUR CHILD:


                                                                                                                               

     1.  OFFICIAL BIRTH CERTIFICATE OR COPY (BIRTH CERTIFICATE WILL BE RETURNED TO YOU.)        

          **NOT A MOTHER’S COPY**

     2.  CHILD’S SOCIAL SECURITY NUMBER (SCHOOL PREFERS A COPY OF THE CHILD’S CARD.)

     3.  PROOF OF COMPLETE PHYSICAL EXAM

            (MUST BE BETWEEN AUGUST 1, 2017 AND AUGUST 1, 2018 PRIOR TO FIRST DAY OF SCHOOL.)

    4.  IMMUNIZATION RECORD WHICH INCLUDES STATE SEAL (FROM EITHER THE HEALTH

         DEPARTMENT OR YOUR CHILD’S HEALTH CARE PROVIDER SHOWING THAT THE CHILD’S IMMUNIZATIONS

           ARE UP-TO-DATE)  **NOT THE BLUE CARD**

       

YOU MUST COME DURING THIS WEEK TO PRE-REGISTER YOUR CHILD IN ORDER FOR YOUR CHILD TO BE TESTED IN MAY.

TIME AND DATE OF TESTING WILL BE GIVEN TO YOU ONLY AT PRE-REGISTRATION.
